You can see Sherlock trying to decide whether to confess.

John says “I actually can’t think of what to say” and Sherlock goes “Yeah, me neither”. 

They have more pauses and jokes and talks about meaningless stuff.

Sherlock keeps looking at John sadly while he’s looking away. 

He’s probably thinking of how this is gonna be the last time he sees John anyway, so he might as well.

As we know, he panics at the last second and says a joke. Seeing John laugh is great at first, of course. Sherlock thinks he made the right choice.

But then he realises he wasted his chance. He’ll have to take the words to his grave. They will remain unspoken.
